<h3><font style="color: rgb(0, 0, 0);">患者王某，男性，22歲，漢族。主因間斷頭疼頭暈伴右側面部感覺麻木1月入院。<br>入院查體：神智清楚，右側顏面部感覺明顯減退，余神經系統查體未見異常。</font></h3> <h3><font style="color: rgb(0, 0, 0);">以上為頭顱CT檢查結果，患者右側中、后顱凹可見巨大腫物占位影。</font></h3> <h3><font style="color: rgb(0, 0, 0);">矢狀位頭顱CT示：鞍旁及橋小腦角區占位</font></h3> <h3><font style="color: rgb(0, 0, 0);">增強MRI平掃及增強顯示中、后顱凹，即鞍旁及橋小腦角區腫瘤。</font></h3> <h3><font style="color: rgb(0, 0, 0);">頭顱CTA顯示腫瘤與周圍血管比鄰情況。</font></h3> <h3><font style="color: rgb(0, 0, 0);">根據患者臨床表現及影像學檢查，入院診斷：三叉神經鞘瘤</font></h3> <h3><font style="color: rgb(0, 0, 0);">三叉神經鞘瘤（trigeminal nerve schwannoma）發生率僅次于聽神經瘤，是第二位的顱內神經鞘瘤，三叉神經鞘瘤約50%位于顱中窩；30%位于顱后窩；20%為啞鈴型，同時累及顱中、后窩。<br>啞鈴型三叉神經鞘瘤會沿著三叉神經走行，由腦干腹側面發出，向上前側方經腦橋小腦角池走向巖頂部。在三叉神經孔處穿過顱中窩的硬膜，位于小腦幕側聯合和巖上竇的下方。小腦后動脈和小腦上動脈跨過其根部上方，小腦前動脈自其下方經過。巖靜脈位于神經根的側方和后方，在巖上竇下方進入三叉神經孔。滑車神經位于腫瘤的上方，第Ⅶ、Ⅷ對腦神經位于腫瘤下方。</font></h3> <h3><font style="color: rgb(0, 0, 0);">完善各項術前檢查以后，于全麻下行三叉神經鞘瘤切除術，患者取側臥位，三釘頭架固定穩妥。</font></h3> <h3><font style="color: rgb(0, 0, 0);">切口位置及形態</font></h3> <h3><font style="color: rgb(0, 0, 0);">骨瓣下緣與顴弓平齊，弧形剪開硬膜，向上翻起。（此為術后補照硬膜切開示意圖）</font></h3> <h3><font style="color: rgb(0, 0, 0);">小心抬起顳葉，釋放腦脊液后，發現此入路腫瘤近在咫尺</font></h3> <h3><font style="color: rgb(0, 0, 0);">腫瘤將小腦幕向上頂出，突出明顯</font></h3> <h3><font style="color: rgb(0, 0, 0);">十字切開腫瘤包膜，先行瘤內減壓。</font></h3> <h3><font style="color: rgb(0, 0, 0);">前部腫瘤徹底切除</font></h3> <h3><font style="color: rgb(0, 0, 0);">向后繼續切開小腦幕，繼續瘤內減壓</font></h3> <h3><font style="color: rgb(0, 0, 0);">可以看到后方的小腦幕緣，及滑車神經。</font></h3> <h3><font style="color: rgb(0, 0, 0);">切開小腦幕，進入后顱凹</font></h3> <h3><font style="color: rgb(0, 0, 0);">發現后顱凹腫瘤體積巨大，張力過高，繼續內減壓</font></h3> <h3><font style="color: rgb(0, 0, 0);">減壓充分后將腫瘤包膜向上翻起</font></h3> <h3><font style="color: rgb(0, 0, 0);">可見橋腦腹側出腦干位置殘留的三叉神經起始部</font></h3> <h3><font style="color: rgb(0, 0, 0);">基本大部切除腫瘤后情況</font></h3> <h3><font style="color: rgb(0, 0, 0);">發現巖骨下方還有少部分腫瘤殘留，有一定的死角</font></h3> <h3><font style="color: rgb(0, 0, 0);">為了徹底的切除腫瘤，高速磨鉆磨除巖骨尖部。</font></h3> <h3><font style="color: rgb(0, 0, 0);">徹底切除后顱凹腫瘤</font></h3> <h3><font style="color: rgb(0, 0, 0);">止血滿意后，鋪填止血材料</font></h3> <h3><font style="color: rgb(0, 0, 0);">縫合手術切口</font></h3> <h3><font style="color: rgb(0, 0, 0);">術后第二天，患者意識清楚，恢復良好，無嚴重并發癥。</font></h3> <h3><font style="color: rgb(0, 0, 0);">術后第二天復查CT情況，腫瘤切除滿意</font></h3> <h3><font style="color: rgb(0, 0, 0);">最后要感謝為此次手術提出過寶貴意見和建議的西京醫院科室的各位老師，還有我的手術團隊的同志們。</font></h3> <h3>患者術后一年復查的效果</h3>
